Well Child Visits
The typical Well Child Visit schedule and procedures done during the visit are as follows:
3-5 days after birth Hepatitis B shot (if not given at birth hospital)
1-2 weeks of life
1 month Hepatitis B #2
2 months Pentacel, Prevnar, Rotavirus
4 months Pentacel, Prevnar, Rotavirus
6 months Pentacel, Prevnar, Rotavirus
9 months Hepatitis B #3; anemia and lead screening
12 months MMR and Varivax
15 months Pentacel and Prevnar
18 months Hepatitis A#1
24 months Hepatitis A #2; anemia and lead screening
3 years
4 years Dtap, IPV, MMR, and Varivax boosters
Yearly, 5-10 years Catch up shots
11 years Adacel, Gardasil, Menactra #1
Yearly, 12-21 years Menactra #2 after 16 y/o
Dr. Dana follows the immunization schedule as recommended by the American Academy of Pediatrics. If there is an alternative vaccination schedule you prefer to follow for your child, please feel free to discuss it at anytime with Dr. Dana.
Well Child Exams are important for many reasons. It is not all about the shots! It is a great time to talk to Dr. Dana about common concerns that come with your child growing up as well as safety, proper nutrition and growth.
If there are any forms that need to be filled out, please give them to the nursing staff before the appointment so they can be ready for you as soon as possible. If the paperwork is brought at the appointment time, our staff will do everything possible to get it back to you at the end of your appointment.
If paperwork is brought at a later date, please allow 24-48 hours turnover time. Dr. Dana reserves the right to charge for excessive paperwork. Typically sports physical forms, daycare forms, and school forms are good for 1 year after a physical. If it has been over one year since your child’s last physical, they will need to come in for a physical before the paperwork can be filled out.
