First Visit

Dr. Dana would like to make your first visit go as smoothly as possible.  Please arrive 15 minutes early to make sure that your paperwork and insurance information are completed.  You can additionally find links to the paperwork so you can have this filled out upon arrival.  If you are bringing in multiple new patients, please consider arriving even earlier.

Failure to show to two new patient visits will result in dismissal from the practice.
